Abstract: 目的 观察地佐辛对ACS患者的临床效果及血浆儿茶酚胺和皮质醇的影响.方法 选取ACS患者62例,发病时间均在12h内,视觉模拟疼痛评分(VAS) ≥4,同期选取20名健康志愿者为对照组.将62例ACS患者按照随机数字表法分两组:地佐辛治疗组(首先静脉注射地佐辛5 mg随后常规药物治疗)和常规药物治疗组,每组31例.每隔10 min观察两组治疗效果,如果VAS评分≥7,静脉注射吗啡3 mg.记录治疗1h内各组出现的心律失常,测定治疗前后VAS评分和血浆心肌肌钙蛋白Ⅰ (cTnI)及儿茶酚胺(肾上腺素、去甲肾上腺素)、皮质醇水平.结果 ACS疼痛患者血浆儿茶酚胺、皮质醇测定值均显著高于对照组(P<0.01).治疗1h后,地佐辛治疗组VAS、cTnl、儿茶酚胺、皮质醇测定值均低于常规药物治疗组(p<0.05或0.01);地佐辛治疗组恶性心律失常发生率低于常规药物治疗组(P<0.05).结论 地佐辛通过有效缓解ACS疼痛及对心脏的保护作用,能抑制应激反应,减轻心肌损伤,减少恶性心律失常发生,使ACS患者临床获益.
