Application of Patient-controlled Analgesia in the Establishment of "Painless Ward"

Abstract: Objective To explore the application of Patient-controlled Analgesia (PCA) in the establishment of "painless ward" in the department of orthopedics.Methods A total of 400 patients with limb fractures meeting the criteria were selected and randomly divided into experimental group and control group. The control group adopted the traditional pain care model, and the experimental group adopted PCA and intervention of painless ward philosophy. Numeric rating scale (NRS) was used to evaluate postoperative resting NRS score and postoperative exercise NRS score. Sleep rate and cases of sleep disruption caused by pain were recorded. In addition, satisfaction level of pain control were recorded.Results Compared with the control group, the experimental group had lower resting NRS scores at each measuring point except 2h after surgery; Exercise NRS scores of the experimental group at each measuring point were lower than that of the control group. At the first postoperative night, patients of the experimental group had higher sleep disruption rate and lower sleep rate than patients of the control group. The satisfaction level of the experimental group was significantly higher than that of the control group, the differences were statistically significant.Conclusions Establishment of painless ward with PCA is a feasible, effective and safe way to help reduce the pain level, carry out rehabilitation exercises and improve quality of life, which is worth to be further applied.
