Causes of Dispensing Mistakes of Medicine Dispensation Machine and Its Improvement

Abstract: Objective Through cause analysis on dispensing mistakes of oral subcontract medicines of SADP, explore management deficiencies of current oral subcontract medicine and develop measures to improve medicine safety management.Methods Took retrospective method and gave full play of QC team. Made use of Excel checklist to count daily oral subcontract medicine errors in pharmacy and 27 clinical departments from July to December in 2014 and analyzed the data.Results Although some errors would occur during the use of SADP, through active improvement measures, oral subcontract medicine error rate dropped from 0.61% in July to 0.18% in March 2015.Conclusion Through continuous improvement on SADP, its error rate has been significantly reduced. Standard operation and routine maintenance could ensure the subcontract accuracy of SADP while strengthening surveillance could ensure clinical medication safety.
