Abstract: Based on the review of its development status at home and abroad, this paper summarized and analyzed the practice of continuing nursing care in Zhejiang Taizhou Hospital and put forward several recommendations on the prospect of implementing H2H continuing nursing care in this hospital as a prefecture-level tertiary hospital.
