Abstract: 目的:探讨呼吸机相关性肺炎(VAP)的病原菌分布及其耐药性,为防治VAP提供科学依据.方法:对214例VAP患者的发病年龄、基础疾病、病原菌分布、耐药率及治疗转归进行回顾性分析.结果:214例VAP患者培养出病原菌293株,其中革兰氏阴性(G-)杆菌203株,占69.3%,革兰氏阳性(G+)球菌62株,占21.2%,真菌28株,占9.5%.其中铜绿假单孢菌居首位;产超广谱β-内酰胺酶(ESBLs)细菌分离率为39.6%;甲氧西林耐药金黄色葡萄球菌(MRSA)占金黄色葡萄球菌总数的54.8%;G-杆菌对常用抗菌药物已表现出较高的耐药率.结论:VAP的主要病原菌为G-杆菌,其耐药率不断增加,应严密动态监测病原菌,合理选用抗生素,加强营养支持以达到有效防治VAP的目的.
