Abstract:
目的观察前交叉韧带(ACL)胫骨附着处的解剖学形态,并对学术界存在的一些相关问题进行探讨.方法测量中国人干燥骨骼标本ACL胫骨附着处的大小、形态、位置.结果最大横径(9.38±1.67)mm;最大纵径(15.93±2.52)mm;附着区前缘距胫骨平台前缘的距离(7.85±1.52)mm;胫骨平台附着区长轴与胫骨平台平面正中矢状轴夹角(21.54±4.61)°;ACL的胫骨附着区形态主要以前宽后窄的倒三角形偏心形态为主(57.4%).结论 ACL胫骨附着处的特殊解剖位置有利于ACL在剧烈运动中具备较强的抗撕脱能力,另一方面亦导致了一旦ACL撕裂,往往会增加合并半月板损伤的发生率;ACL纤维在胫骨附着处的不规则偏心分布形态为膝关节镜下ACL重建寻找"等长点"带来困难.
