Abstract: 目的探讨颈段食管手术治疗效果.方法回顾性分析手术治疗颈段食管癌29例,其中食管入口处8例,胸廓入口处9例,25例采用食管切除,胃代食管术.左颈食管胃吻合17例,胃下咽吻合8例,同期行全喉切除,气管造口2例.双侧颈阔肌皮瓣管代食管2例,同期全喉切除.胸大肌皮瓣管代食管2例,同期全喉切除1例.颈胸腹三切口切除共18例,颈腹二切口不开胸食管内翻拨脱术7例.颈淋巴结清扫4例,胃经食管床上提至颈部吻合.结果病理检查29例均为鳞状细胞癌,行一期手术切除,1年生存率72.0%,3年生存率41.0%,5年生存率17.0%.结论颈段食管癌应采取积极的手术治疗.
