Coronary CT Angiography with Optimizing Contrast Medium Based of Body Mass in CT

Abstract: 目的 探讨128层双源Flash CT大螺距冠状动脉成像低对比剂剂量优化时的可行性.方法 将61例行冠状动脉CTA检查的患者随机分为两组(A组30例,B组31例),按照对比剂剂量和体质量的比值分别为0.7ml/kg和0.5 ml/kg,各组的对比剂剂量等于体质量乘以各自的比值.测量肺动脉、主动脉、左心房、左心室、冠状动脉各分支血管以及心肌下壁的CT值.采用独立样本£检验评价两组血管、心腔及心肌下壁CT值、对比剂剂量和有效辐射剂量,采用x2检验比较两组间冠状动脉图像质量分级显示段数.结果 两组中各冠状动脉主干及其分支血管清晰显示,对比剂剂量分别为(47 ±8)ml、(36±6)ml,差异有统计学意义(t=5.343,P=0.020).升主动脉CT值分别为(454 ±61)HU、(439±77) HU,差异无统计学意义(t=-0.794,P=0.430).A、B两组冠状动脉分别有374与386段,评分为Ⅰ级与Ⅱ级的段数分别有368段和378段,所占总冠状动脉段数的百分比分别为98.4%和97.9%,差异无统计学意义(x2 =0.230,P=0.631).辐射剂量分别为(0.88±0.04) mSv、(0.85 ±0.07) mSv,差异无统计学意义(t=2.087,P=0.087).结论 128层双源CT冠状动脉成像在采用对比剂剂量和体质量之比为0.5ml/kg时,既可以满足临床诊断,又可以最大程度的降低对比剂剂量.
