Abstract: Objective:To explore the anesthesia management of emergency caesarean section for pregnant women with heart failure. Methods:A retrospective analysis of 15 cases of pregnancy with heart failure for urgent cesarean section was in-vestigated,whose heart function at level Ⅲ~Ⅳ. All the operations were performed under intravenous general anesthesia with endotracheal intubation. Anesthesia was maintained with propofol and remifentanil after fetal delivery. The changes of mean ar-terial pressure( MAP),heart rate( HR)and pulse oxygen saturation( SpO2 )were observed before anesthesia,at the begin-ning of the operation,fetal delivery and at the end of anesthesia. Apgar scores at 1,5 min after baby delivery were recorded. Maternal and neonatal outcomes were evaluated. Results:All cases underwent cesarean section successfully. Maternal MAP was lower after fetal delivery when compared with baseline before anesthesia(P﹤0. 05). There was no significant difference in HR during operation(P﹥0. 05). After the beginning of the operation,SpO2 was significantly higher than that before anesthesia(P﹤0. 05). Acute or chronic fetal distress exists in 5 cases of pregnant women before operation. One case of abortion was a very low birth weight infant,and further treatment was given up;Average Apgar scores of 16 cases of full-term and premature in-fants at 1,5 minutes after baby delivery were(7. 13 ± 1. 67)and(8. 75 ± 1. 00)respectively. Conclusion:Intravenous general anesthesia with propofol and remifentanil can apply to the pregnant women with heart failure safely.
