Systematic review of effects of truncal ligation of inferior thyroid arteries on parathyroid function during subtotal thyroidectomy

Abstract: 【Objective】To assess the effects of the truncal ligation of inferior thyroid arteries on parathyroid function during subtotal thyroidectomy.【Methods】A total of 116 patients with thyroid disease treated in our hospital from March 2013 to March 2015 were randomly divided into observation group and control group according to the order of admission, with 58 cases in each group. Two groups were given subtotal thyroidectomy, observation group was given the truncal ligation of inferior thyroid arteries, while the control group was not. The incidences of complications, hypoparathyroidism, hypocalcemia(serum calcium levels and serum total calcium levels were compared respectively on the fi rst day and third day after operation) and low calcium symptoms, operation time, intraoperative blood loss, analgesic usage and hospitalization time of two groups were compared.【Results】After surgery, the operation time and length of hospital stay of the observation group were signifi cantly shorter, and the intraoperative blood loss, amount of analgesic of the observation group were signifi cantly lower than those of the control group(P <0.05). The differences in the incidences of hypoparathyroidism, hypocalcemia and low calcium symptoms between the two groups had no statistical signifi cance(P >0.05). The incidences of various complications and total incidence of complications of the observation group were signifi cantly lower than those of the control group(P <0.05). The serum calcium levels and serum total calcium levels on the fi rst day and third day after surgery between the two groups were not statistically different(P >0.05).【Conclusion】Compared with subtotal thyroidectomy without truncal ligation of inferior thyroid arteries, the truncal ligation of the inferior thyroid arteries during subtotal thyroidectomy has no effect on parathyroid function and serum calcium, but, it can reduce the incidence of complications, shorten the operation time and length of hospital stay, and improve the quality of life of patients after surgery.
