EFFECT OF POSITIVE END-EXPIRATORY PRESSURE ON INTRA-ABDOMINAL PRESSURE IN ADULT PATIENTS WITH ACUTE RESPIRATORY DISTRESS SYNDROME:A META-ANALYSIS

Abstract: Objective To compare the effects of high and low positive end-expiratory pressure(PEEP)on intra-abdominal pressure(IAP)in patients with acute respiratory distress syndrome(ARDS). Methods A full search of databases of PubMed,Embase,Ovid,CBM,and CNKI-from 1998 to 2015-was done.The clinical research papers,in English and Chinese,concerning the effect of PEEP on IAP in ARDS patients were collected,the papers were selected and data extracted according to the inclusion and exclusion criteria.A meta analysis was carried out using RevMan 5.3. Results A total of six papers concerning observation and research were included,in which,101 cases of ARDS patients enrolled.Compared with low PEEP,the IAP level in ARDS patients with high-PEEP was higher(SMD=0.76,95%CI=0.29-1.23,P<0.05),but high PEEP did not change the heart rate and mean arterial pressure in patients with ARDS(P>0.05). Conclusion Under the condition of the other settings of the ventilator parameters remain unchanged,mechanical ventilation set at high positive end-expiratory pressure may increase intra-abdominal pressure in patients with acute respiratory distress syndrome,but,in a short time,there will be no significant effect on patient’s heart rate and mean arterial pressure.
