Clinical Efficacies of Different Doses of Oxycodone in Prevention of Stress Response during Double-Lumen Endotracheal Extubation

Abstract: ABSTRACT:Objective To observe the clinical efficacies of different doses of oxycodone in the prevention of stress response during double-lumen endotracheal extubation.Methods Eighty pa-tients undergoing selective thoracotomy and double-lumen endotracheal intubation under general anesthesia were randomly divided into four groups:0.1 mg· kg-1 oxycodone treatment group (group OL),0.15 mg·kg-1 oxycodone treatment group(group OM),0.2 mg·kg-1 oxycodone treatment group(group OH)and control group(group C).Different doses of oxycodone injection were administered at 5 minutes before the end of surgery.The control group received the same volume of normal saline.The operation time,intraoperative fentanyl and remifentanil consump-tion,postoperative spontaneous breathing recovery time,extubation time,and adverse reactions (nausea,vomiting,drowsiness and respiratory depression)were recorded in all groups.Further-more,heart rate(HR),systolic blood pressure(SBP)and diastolic blood pressure(DBP)were measured immediately after transfer to recovery room(T0 ),immediately after extubation(T1 ), 3 minutes after extubation(T2 ),and 5 minutes after extubation(T3 ).Plasma cortisol concentra-tions were detected by radioimmunoassay.Results Compared with T0 ,the HR,SBP,DBP and cortisol levels were increased at T1-T3 in group C and group OL(P <0.05),but were not changed in group OM and group OH(P >0.05).Compared with group C,the HR,SBP,DBP and cortisol levels were decreased at T1-T3 in group OM and group OH,the postoperative spontaneous breath-ing recovery time,extubation time and incidences of drowsiness and respiratory depression were increased in group OH(P <0.05),but the incidences of adverse reactions were not changed after extubation in group OL and group OM(P > 0.05).Conclusion Intravenous administration of 0.15 mg·kg-1 oxycodone hydrochloride at 5 minutes before the end of surgery can effectively re-duce stress response and adverse reactions during double-lumen endotracheal extubation without affecting the recovery time.
