Effect of memantine combined with quetiapine on behavioral psychological symptoms of dementia in vascular dementia

Abstract: Objective To evaluate the effect of memantine combined with quetiapine on behavioral psychological symptoms of dementia (BPSD) in vascular dementia.Methods Totally 79 cases of patients of vascular dementia with BPSD were randomly divided into control group and observation group;there were 39 cases in control group and 40 cases in observation group.Quetiapine was orally given before sleep in both groups [initial dose 25 mg/time,50 300 mg,average (100 ± 46)mg];memantine was additionally administrated in the morning [5 mg/d at the 1st week,then 10 mg/d];the treatment was lasted for 12 weeks.The rating scale of the behavioral pathology in Alzheimer's disease (BEHAVE-AD),minimum mental state examination (MMSE) and activities of daily living (ADL) were assessed before and 2,4,8,12 weeks after treatment;the treatment emergent symptom scale (TESS) was used to evaluate the adverse reactions.Results The BEHAVE-AD,ADL,MMSE score before treatment and 2,4,8,12 weeks after treatment in control group were as followed:BEHAVE-AD score:(18.1 ± 3.2),(17.6±5.2),(16.0±5.4),(15.2±4.7),(13.8±3.2) scores,MMSE score:(15.1±4.5),(16.1 ± 3.2),(17.0±2.4),(18.2±3.8),(18.9±2.8) scores,ADL score:(44.1 ±3.2),(42.3 ±4.3),(39.1 ± 2.1),(37.2 ±5.3),(34.5 ±4.2) scores;in observation group were as followed:BEHAVE-AD score:(18.6 ± 5.5),(17.0±5.2),(15.5 ±3.2),(13.6 ±3.2),(10.4±2.2)scores,MMSE score:(15.1 ±5.3),(16.2± 4.2),(18.7±4.4),(21.2±4.8),(23.2±4.8) scores,ADL score:(44.6 ±5.8),(41.2±5.4),(38.1 ± 5.6),(34.5 ±5.7),(30.5 ±4.2) scores.Compared with those before treatment,the scores of BEHAVE-AD and ADL were significantly reduced 2,4,8,12 weeks after treatment in observation group,4,8,12 weeks after treatment in control group (P <0.05 or P <0.01);the MMSE score were significantly increased 4,8,12 weeks after treatment in observation group,8,12 weeks after treatment in control group (P < 0.05 or P < 0.01);the scores of BEHAVE-AD and ADL were significantly lower,the MMSE score was significantly higher in observation group than those in control group 8 and 12 weeks after treatment (P < 0.01 or P < 0.05).The incidence of adverse reactions and TESS scores were not significantly different between groups (P > 0.05).Conclusion Memantine combined quetiapine can improve the BPSD more effectively compared with single quetiapine with good safety.
