Abstract: Objective To investigate the clinical effect of total hip replacement arthroplasty in the treatment of femoral neck fracture in elders. Methods 96 cases of femoral neck fracture in elders from February 2011 to September 2014 were selected and randomly divided into control group and treatment group(n=48).The patients in control group were treated with artificial femoral head replacement, while the patients in treatment group were treated with total hip replacement arthroplasty. The clinical effect, operation time, intraoperative blood loss, time of off- bed, amount of post- operation drainage, postoperative complication, the changes of life quality and Harris scores of the patients were recorded and compared.Measurement data was processed by t test, count data was processed by chi square test,P<0.05 was considered statistically significant. Results After treatment, the excellent and good rate of treatment group was 89.58%,higher than that of the control group(70.83%),the difference was statistically significant(P<0.05).The operation time, intraoperative blood loss, time of off- bed, amount of post- operation drainage of treatment group were(79.36±15.64)min,(573.18±68.04)ml,(15.38±1.69)d,(159.17±30.62)ml,superior than that of the control group [(71.72±20.49)min,(352.56±41.52)ml,(21.97±2.46)d,(101.42±23.58)ml],the differences were statistically significant(all P<0.05).The occurrence rate of complication of treatment group was 8.33%,lower than that of the control group(27.08%),the difference was statistically significant(P<0.05).The physiological function, social function, affection function and Harris scores of treatment group were(76.58±5.28),(75.08±5.68),(75.24 ±5.58),(92.84 ±1.57)points,higher than that of the control group [(71.94 ±3.92),(69.59 ±4.27),(70.93 ±5.06),(81.28±0.95)points],the differences were statistically significant(all P<0.05).Conclusion The use of total hip replacement arthroplasty has good effect in the treatment of femoral neck fracture in elders, worthy of clinical application.
