Early intellectual development and its risk factors of late preterm infants

Abstract: Objective To study early intellectual development and its risk factors of late preterm infants(gestational age from 34 to 36 weeks).Methods Late preterm infants born in our obstetric ward and admitted to our neonatal ward between April 2013 and March 2014 were enrolled in our study.Fullterm infants matched with late preterm infants through pairing of multiple social factors were studied as control.At 40 weeks,6 months and 1 year of corrected age,intelligence level of infants were assessed and compared.According to intelligence evaluation results at corrected age of 1 year,the late preterm infants were assigned into normal group and study group,and clinical factors that may affect neurological development were analyzed.Results Total of 50,48,and 45 late preterm infants were assessed at40 weeks,6 months and 1 year of corrected age,respectively.Meanwhile,60,51,and 46 full-term infants were also evaluated in the corresponding time.At corrected age of 6 months,the development of gross motor skills of infants born late preterm were lagging behind their peers of term birth;at corrected age of 1 year,the development of gross motor skills,fine motor skills,and language skills in late preterm infants remain lagging behind term infants,the differences were both statistically significant(P < 0.05).Multivariate analysis showed that small for gestational age infants(OR =7.361,95%CI:4.937-10.971),hypoglycemia(OR =4.768,95%CI:1.325-17.162),neonatal respiratory distress syndrome(OR=4.621,95%CI:2.022-10.557),and low education level of parents(OR = 1.930,95%CI:1.524 — 2.363) were risk factors for retardation of early intellectual development in late preterm infants.Conclusions Early intellectual development of late preterm infants lag behind term infants.Small for gestational age infants,hypoglycemia,neonatal respiratory distress syndrome,and low education level of parents are risk factors.It is necessary to improve the observation of late preterm infants and carry out targeted intervention.
