Abstract: 目的探究多种生化指标联合检测预测动脉粥样硬化斑块稳定性的可行性。方法根据患者粥样斑块是否稳定分为稳定组(95例)及非稳定组(301例),用免疫散射比浊法、酶联免疫吸附法检测两组患者超敏C-反应蛋白(hs-CRP)、基质金属蛋白酶(MMP-9)、单核细胞趋化因子(MCP-1)三种指标的浓度。结果与非稳定组相比,稳定组hs-CRP、MMP-9、MCP-1的浓度更低,差异具有统计学意义(t=43.500,t=15.609,t=18.396;P均<0.01);三种物质的最佳检测浓度分别hs-CRP为8.35 mg/L,MCP-1为0.282pg/mL,MMP-9为0.141ng/mL,在最佳检测浓度下,稳定组中MMP-9的敏感性33.4%,特异性为86.2%,hs-CRP的敏感性92.3%,特异性为77.2%,MCP-1的敏感性44.3%,特异性为75.3%;非稳定组中MMP-9的敏感性71.3%,特异性为85.6%,hs-CRP的敏感性34.6%,特异性为62.3%,MCP-1的敏感性80.2%,特异性为88.9%。以hs-CRP>8.35 mg/L,MCP-1>0.282pg/mL,MMP-9>0.141ng/mL为标准诊断稳定组的敏感性和特异性提高到97.4%、92.9%,非稳定组的敏感性和特异性提高到94.0%、95.5%。结论 hs-CRP、MMP-9、MCP-1联合检测可以提高对动脉粥样斑块稳定性预测的敏感性和特异性。
