Comparison of the effect of continuous intravenous infusion of high dose methotrexate by infusion pump and syringe pump

Abstract: Objective To summarize the effect of continuous intravenous infusion of high dose methotrexate by infusion pump or syringe pump retrospectivly.Methods 110 children with acute lymphoblastic leukemia（ALL） received 440 courses of high dose methotrexate（HDMTX） during consolidation therapy from Sep.2012 to Oct.2014.Peripherally inserted central venous catheters（PICC）were indwelled for all patients before chemotherapy.From Sep.2012 to Aug.2013,the continuous intravenous infusion of HDMTX was done with infusion pump.From Sep.2013 to Oct.2014,it was done with syringe pump.The completion time of HDMTX,the 24 h and 48 h plasm MTX concentrat ions（MTX-24 h,MTX^l-8 h） after the start of HDMTX and MTX-associated toxicity that was graded according to national cancer institute-common toxicity criteria version 3.0（NCI-CTC 3.0） were analyzed retrpspectivly.Results The exact completion time（ECT） of HDMTX infusion was the 24 th hours after the start of HMTX,the permitted normal range was from the 23rd to 25th hours.The courses that HDMTX was finished on ECT of syringe pump group were obviously more than that of infusion pump group（P <0.05） and the bias of ECT of the former was obviously less than that of the latter（0.50 h ± 0.28 h and 1.90 h ± 1.11 h,respectively）.The MTX-24 h of syringe pump group was nearer the plasm stable concentration of MTX（P < 0.05）,but the MTX-48 h of the former was obviously lower than that of the latter and the incidence rates of MTX- associated toxicity of syringe pump group were obviously less than that of infusion pump group（P < 0.05）.Conclusions HDMTX infusion with syringe pump via PICC was so accurate,effective and safe that it was worthy popularization in clinical application.
