Laparoscopic Surgery for Vaginal Endometriosis:a Clinical Analysis of 43 Cases

Abstract: Objective To analyze the safety and efficacy of laparoscopic surgery for the treatment of vaginal endometriosis . Methods From January 2002 to June 2013, 43 patients undergoing laparoscopic surgery with pathology confirmed vaginal endometriosis were analyzed retrospectively . According to the principles of endometriosis surgical treatment , the patients were performed by conventional laparoscopy examination , water cushion injection , dissociation of rectum , bladder pushdown , dissociation of ureters, excision of endometriotic lesions , rectum completeness test , incision suture and drainage tube placement . Results Among the 43 patient, 32 patients received laparoscopic excision of endometriotic lesions with adhesiolysis (conservative surgery), 6 patients received laparoscopic excision of endometriotic lesions with hysterectomy ( semi-radical surgery ) , while the rest 5 patients received laparoscopic excision of endometriotic lesions and hysterectomy with oophorosalpingectomy ( radical surgery ) .A total of 36 patients were followed up for 6 -126 months, with a median of 43.7 months.Among the 26 cases treated with conservative surgery , the remission rate of dysmenorrhea, anus bulge, chronic pelvic pain, dyspareunia, and vaginal bleeding after operation were 77.8%(14/18), 50%(4/8), 60%(3/5), 100%(5/5), 100%(8/8), respectively;endometriosis recurred in 3 cases after operation, with the recurrence time being 1, 5, 8 months, respectively.Among the 5 cases of semi-radical surgery and 5 cases of radical surgery , 8 cases of dysmenorrheal , 5 cases of anus bulge and 1 case of vaginal bleeding had significant remission;1 case of chronic pelvic pain achieved significant remission and 1 achieved partial remission; endometriosis recurred in 1 case 3 months after operation .Of the 8 infertile patients , 4 became pregnant within 1 year after operation and gave live birth . Conclusion Laparoscopic surgery is safe and effective for vaginal endometriosis .
