Abstract:
作者介绍1例16岁少女,心脏手术后合并脑栓塞,近于脑死亡状态,从发病24小时应用轻度低温疗法。发病2日、4日CT有明显脑水肿,但血脑屏障功能已有所恢复,此后脑水肿明显减轻,出院后高校复学。低体温对大脑具有重要的保护作用,但以前为获得大脑的保护效果,至少要把体温降至30℃以下,容易引起心功能抑制和重症心律失常,很难在临床应用。80年代有人提出32~33℃的体温对保护大脑取得满意效果。轻低温可抑制脑的能量代谢,减轻酸中毒,抑制自由基的产生,减少兴奋性氨基酸的释放,使脑出血和头部外伤症状明显减轻。33℃的脑温对心功能基本无影响,可在重症监护室(ICU)监护。但脑温低于32℃可引起频发室性期外收缩,这
