Abstract: <正> 答案一起搏器介导的心动过速(PMT)频率为136 bpm,由未下传的房性早搏(PAB)终止。由于完全性室房(VA)传导伴长VA传导时间,以致于心房起搏的不应期后才出现逆行P波。每一QBS波后跟随的心房活动(如图1箭头示),被起搏器心房电极感知,触发心室起搏,心室起搏后又逆传入心房,再被感知后,触发心室起搏,如此循环不已,形成PMT。故此心动过速是心房感知、触发心室所致。这种心动过速的诊断应考虑窦性心动过速(简称窦速)、房性心动过速(简称房速)和PMT。但窦速或房速可通过P波的形态区别于PMT。PMT在下肢导联有倒置P波。窦速时,颈动脉窦按压可降低窦率和心室起搏频率,房速则无反应。因为按压颈动脉窦可产生VA传导阻滞,PMT则可被终止。磁铁试验可使DDD起搏器转
