Indications selection and operation time of micro-invasive drainage for traumatic extradural hematoma

Abstract: Objective To evaluate the curative effects,indications,operation time of micro-invasive drainage for traumatic extradural hematoma.Methods According to the volume of hematoma and GCS points of patients,one hundred and thirty six cases of traumatic extradural hematoma were divided into four groups:micro-invasive drainage Ⅰ group(36 cases,the volume of hematoma ≥30ml), craniotomy group (32cases,the volume of hematoma ≥30ml), micro-invasive drainage Ⅱ (32cases,the volume of hematoma <30ml)and the conservative group (36cases,the volume of hematoma <30ml); their curative effects, complications, the hospitalization time and hospitalization cost were compared respectively.Results The 136 cases got an excelllent prognosis.8 cases was ineffective in micro-invasive drainage Ⅰ group, then their hematomas were cleaned by craniotomy;7 were subjected from sequela of skull of defect and mild hemiplegia in craniotomy group after operation;3 from a postponed epilepsy in the conservative group.There was not significant difference between micro-invasive drainage groups and control groups,but sequela,hospitalization time and cost in former were low than in control groups (P<0.05).ConclusionMicro-invasive drainage operation is safe,effective and minimally invasive for traumatic extradural hematoma.It is a key to success of operation that indications and time of operation must be strictly selected.
