Abstract: 胸水是肺部多种疾病的常见并发症,其中绝大多数恶性胸水是由肺腺癌引起.常规细胞学检查主要根据细胞形态进行诊断,由于腺癌细胞与反应性间皮细胞形态极为相似,故鉴别以上两种细胞是诊断恶性胸水的最大难点.甲状腺转录因子1(TTF-1)特异表达于肺腺癌、小细胞肺癌和甲状腺癌,在排除极少发生甲状腺癌的前提下,通过检测TTF-1既有助于诊断肺癌性胸水,又可区分肿瘤的来源是原发于肺还是继发于肺,以此为临床治疗提供新靶点.
