The clinical studies on KatG genic mutation of mycobacterium tuberculosis and isoniazid's resistance degree

Abstract: 目的 探讨结核分枝杆菌KatG基因突变与对异烟肼耐药程度的关系.方法 对临床分离的61株结核分枝杆菌异烟肼耐药株(其中高浓度耐药株49株,低浓度耐药株12株)和42株结核分枝杆菌异烟肼敏感株进行DNA序列分析.结果 61株异烟肼耐药株中,检测到50株存在KatG S315T(AGC→ACC)基因突变,1株存在Ser315Arg(AGC→AGA)的突变,1株发生Val319Asp(GTC→GAC)突变,突变率为85.2%(52/61).其中高浓度耐药株中有46株检出发生S315T突变,1株发生Val319Asp突变,突变率为95.9%(47/49);低浓度耐药株中有4株检出发生S315T突变,1株发生Ser315Arg(AGC→AGA)的突变,突变率为41.7%(5/12);42株异烟肼敏感株DNA序列分析均没有检测到KatG基因突变.结论 KatG基因突变是结核分枝杆菌耐异烟肼的主要分子机制,异烟肼高浓度耐药株的KatG基因突变率远高于低浓度耐药株的基因突变率.
